Job Description
A state entity has an immediate need for experienced case managers to assist with a health services program.
Responsibilities:
- Provide on-going intense case management services to 45 + clients in a private home or assisted living setting.
- Perform in-home and assisted living setting assessments to evaluate the medical and social needs of participants.
- Conduct face-to-face interviews with participants, responsible party, or service provider to include diagnosis, prescribed medications and treatments, functional, cognitive, behavioral, and health status.
- Evaluate the appropriateness and effectiveness of waiver and non-waiver services.
- Evaluate, record, and change the participant's general health and well-being.
- Review hospital discharge plans.
- Complete designated program client assessment forms.
- Pre-authorize waiver services and coordinate service providers.
- Assign OTP devices to clients without landline phones for providers to utilize for clocking in when providing services.
- Check ISAS (In-home Supports Assurance Systems).
- Review nurse monitor reports.
- Follows up with Nurses to ensure visits for timely.
- On-going monitoring of the client's medical condition, monitoring services delivery, advocating on behalf of the clients with the client's physician, insurance companies and other social service agencies/programs.
- Documents on case activity in the progress note section of the client's chart.
- Prepares a written report of the client assessments.
Job Requirements
- Ability to learn and utilize Maryland LTSS (Long Term Support Services) for day-to-day operations.
- MS Word and Access skills.
- Good Client assessment skills.
- Strong communication skills (verbal and written).
- Ability to read & interpret regulations.
- Knowledge of medical terminology.
- One year of Human Service experience providing community based care management services to the elderly along with a Bachelor's Degree in Human, Psychology or Social Services is required.
- Valid driver's license, own vehicle and good driving record is required.
